Job Summary Highland Rivers Behavioral Health Intensive Case Management (ICM) Team is a medium‑intensity community‑based program for adults with severe and persistent mental illnesses such as bipolar disorder, schizoaffective disorder, and other thought disorders.

Responsibilities
Develop an Individual Recovery Plan (IRP) that includes medical, behavioral, social, educational, vocational, housing, financial, and other needs.
Provide transportation for individuals and visit their homes as required.
Assist individuals with service needs, develop strategies, and coordinate rehabilitation services.
Partner with individuals and CORE services providers in the development of the IRP.
Advocate and link with other agencies to secure needed resources.
Provide assistance and support during crisis situations.
Perform assessments, monitoring, and interventions for individuals.
Complete all necessary paperwork according to policy and ensure eligibility criteria are met.
Manage a caseload by ensuring all services are authorized, charts are current, and cases are closed per policy.
Maintain weekly contact and productivity standards; attend treatment team meetings as directed by Program Supervisor.
Assist with service‑related compliance and maintain confidentiality per agency and HIPAA policies.
Perform other related responsibilities and tasks as assigned.
