Job Summary and Responsibilities
Pharmacist – St. Bernardine Medical Center

St. Bernardine Medical Center is seeking a skilled and detail-oriented Pharmacist to provide comprehensive pharmaceutical care across inpatient services. In this vital role, you will ensure safe medication practices, participate in performance improvement initiatives, and serve as a trusted resource for clinical staff and providers.
Key Responsibilities
Review, process, and dispense medications and IV therapies with precision and in compliance with all hospital policies and regulatory guidelines
Provide direct clinical consultation and medication therapy recommendations to physicians and nursing teams
Supervise and support pharmacy technicians in compounding, repackaging, and medication distribution
Monitor patient medication profiles for drug interactions, allergies, and contraindications
Perform pharmacokinetic calculations and adjust dosages as appropriate for patient age, weight, and condition
Support formulary compliance, therapeutic interchange, and clinical pharmacy initiatives
Participate in department performance improvement projects, medication use evaluations, and adverse drug reaction monitoring
Ensure accurate and timely documentation within the EMR and pharmacy systems
Maintain compliance with controlled substance protocols, regulatory standards, and documentation requirements
Job Requirements
1-2 years experience in Hospital Pharmacy, or completion of a Pharmacy Residency program preferred
Experience in unit-dosing, IV admixtures, pharmacokinetics and TPN desired
Graduate of a School of Pharmacy, required Pharm D; desirable if available
Basic knowledge of computers desirable
California Pharmacist License required
