Role Summary The Qualified Intellectual Disability Professional II (QIDP II) is central to the overall responsiveness and effectiveness of an individual’s active treatment program at a State Supported Living Center (SSLC). This role oversees the Individual Support Plan (ISP) process, coordinates interdisciplinary team (IDT) meetings, and ensures compliance with federal and state standards governing Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/IID).
Responsibilities
Oversee and document the ISP process for all individuals on the assigned caseload, including facilitation of IDT meetings, ISP Addendum meetings, and annual ISPs.
Coordinate and facilitate the participation of individuals, their legally authorized representatives, and staff in the development and delivery of needed supports and services.
Ensure timely follow‑up on all recommendations for supports, services, equipment, or programs by collaborating with IDT staff.
Collect and analyze data on ISP implementation and effectiveness; document findings in electronic records and the monthly QIDP review documents.
Train and coach staff in the implementation of services, supports, and teaching methodologies, providing feedback to promote competent interactions with individuals.
Monitor and evaluate the implementation of biomedical, behavioral, and environmental supports, ensuring alignment with individual goals and behavioral health needs.
Serve as an advocate for the individual, ensuring proper client protections, timely response to significant life events, and adherence to the Human Rights Committee (HRC) process.
Participate in agency obligations for disaster response, continuity of operations, and other duties as assigned.
Essential Job Functions
Attends work on a regular basis and may be required to work a specific or rotating shift schedule, including extended hours or overtime in accordance with agency leave policy.
Monitors and evaluates ISP plans, programs, supports, and services on a monthly basis.
Provides leadership and ad‑hoc training to staff on specific issues related to assigned individuals.
Knowledge, Skills & Abilities
Knowledge of the IDT process and people‑centered planning.
Understanding of rehabilitation and skill acquisition programs for persons with intellectual disabilities.
Ability to coordinate programmatic and clinical services and supports.
Strong communication skills, both verbal and written.
Problem‑solving and independent work with strong prioritization.
Adaptability to change and ability to work harmoniously with staff at all levels.
Physical capability to perform direct care work, including emergency procedures.
Proficiency with computer software, electronic records systems, and database reporting.
Initial Screening Criteria
Bachelor’s degree in psychology, sociology, social work, special education, rehabilitation counseling, therapeutic studies, or a related human services field.
At least one year of experience working or volunteering directly with persons with intellectual or developmental disabilities.
Additional Information
Applicants must pass a fingerprint criminal background check, pre‑employment drug screen, and registry checks.
All applicants must be at least 18 years of age to be considered for employment at a state‑operated facility.
Flexibility in work hours may be required; the position may require overtime, extended hours, weekends, and holidays.
Random drug testing is required for all State Supported Living Center employees.
