The Community Response case manager works collaboratively with law enforcement agencies and representatives of Highland Rivers Behavioral Health on a community response team. The team focuses on diverting individuals from jails and emergency departments, reducing non‑emergency 911 calls, and carrying a caseload of at‑risk individuals until they are fully engaged in services at the most appropriate level of care. The case manager facilitates linkage to ongoing treatment, keeps the team therapist abreast of any new developments in the caseload, and may provide follow‑up visits for individuals who have experienced an overdose. Responsibilities

Provide follow‑ups as a member of the community response team to coordinate behavioral health wrap‑around services for individuals who have recently accessed 911 for behavioral health needs. Assess immediate needs and assist individuals in identifying appropriate resources. Identify social and community service agency options and make appropriate recommendations. Work closely with the team therapist to manage a caseload of individuals in need of wrap‑around services. Collaborate with individuals and team therapist to formulate person‑centered behavioral health plans. Coordinate community follow‑up contacts with individuals post‑crisis. Attend staffing/case meetings with team members as required. Adhere to all documentation standards and requirements. Educate community partners about available programs and services. Attend community events to raise local awareness of services. Perform other duties as assigned. Benefits
