is seeking a compassionate and dedicated
Medical Social Worker
to provide emotional support, counseling, and resource coordination to patients and families. The Medical Social Worker plays a key role in assisting individuals to cope with the social, emotional, and financial challenges associated with illness, hospitalization, or ongoing medical treatment.
Job Responsibilities
Conduct psychosocial assessments to evaluate patient needs, strengths, and support systems. Develop individualized care plans addressing emotional, social, and environmental factors affecting patient well-being. Provide counseling and crisis intervention to patients and families coping with illness, loss, or life transitions. Assist patients and families in accessing community resources, financial assistance programs, and social services. Collaborate with physicians, nurses, and interdisciplinary team members to ensure holistic and coordinated care. Facilitate patient discharge planning, ensuring appropriate post-hospital care and support arrangements. Document all social work interventions, care coordination activities, and progress notes in the electronic health record (EHR). Advocate for patients' rights and ensure culturally sensitive care delivery. Participate in team meetings, case conferences, and quality improvement initiatives as needed. Qualifications
Master's degree in Social Work (MSW)
from an accredited program required. Current state licensure
as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) required, depending on state regulations. Minimum of 1 year of experience in a healthcare, hospital, or community health setting preferred. Knowledge of medical terminology, discharge planning, and community resources. Strong communication, advocacy, and problem-solving skills. Ability to provide compassionate, patient-centered support in sensitive or stressful situations. Benefits
