Community Health Alliance is looking for a Population Health Supervisor. Join our talented team of Population Health Management that focus on serving the community and helping those in need by creating healthy outcomes for patients of every income.

We operate six convenient locations throughout Reno and Sparks, providing comprehensive, top-quality medical services, dental and pediatric care, behavioral health, low-cost pharmacies, nourishing food pantries, and more.

Job Summary/Objective

Supports the Population Health team with Community Health Alliance's (CHA's) program developments, project management, and value-based health plan relationships. Provides supervision of the Healthcare Access Specialist team, team outreach projects, and associated Population Health staff. Primary objectives and expectations are to provide oversight of the Healthcare Access Specialists team members, patient outreach management, support the Population Health team on value-based program and contract compliance, program needs/risk assessment, data analysis, and work interdepartmentally with a variety of CHA staff and community partners to meet and exceed program goals.

Required Knowledge, Skills & Abilities:

Demonstrates strong supervisory skills, evaluates performance efficiency, effective communication, time management, and conflict resolution skills. Demonstrates experience managing multiple complex initiatives across organizational platforms in close collaboration with leaders. Knowledge of the essential functions, practices and procedures of a medical clinic and office. Strong knowledge of Medicaid, Medicare, and Commercial health plan programs. Skilled with Excel, PowerPoint, Microsoft Office, and project management tools. Skilled in navigating through provider portals and accessing appropriate information. Strong demonstrated experience in report development, report management, and ability to prioritize projects based on data generated in a report. Knowledge of medical billing/coding, CPT/ICD, and medical terminology. Skill in accuracy, attention to detail, organization and project timeline management. Ability to maintain confidentiality of financial and medical information according to Health Insurance Portability and Accountability Act (HIPAA) regulations. Knowledge of National Committee for Quality Assurance (NCQA) Healthcare Effectiveness Data and Information Set (HEDIS). Ability to solve practical problems and identify pathways to resolve risks and barriers. Ability to exercise good judgment in appraising situations and making decisions. Ability to work and interact effectively and positively with other staff members to build and enhance teamwork. Ability to travel between health centers and outreach events as needed. Strong ability to communicate in a courteous and professional manner. Ability to understand and respond appropriately, effectively and sensitively to special population groups as defined by race, ethnicity, language, age, sex, etc. Ability to hear and speak to converse over the telephone and in-person in a professional and respectful manner. Any additional duties as assigned by Community Health Alliance, Director of Population Health Management.

Collaboration:

Clinical leadership, Medical, Dental, Pharmacy and Behavioral Health providers, IT department, MA's and MA Supervisors, Health Center Managers and Site Supervisors, Care Coordination, Community Health Workers, Call Center Representatives, and Community partners.

Directly reporting personnel:

Healthcare Access Specialists, Population Health Coordinator(s), and Population Health Outreach Specialist(s).

Education and Experience:

Bachelor's degree in related field (e.g. Healthcare Administration, Public Health, Social Work, Health Sciences) or equivalent experience required. Minimum of four (4) years of relevant experience required and minimum of two (2) years medical experience required. Experience with eClinicalWorks (Electronic Medical Records), patient chart review, project management, HEDIS, data analytics, patient outreach, health plan portals (Availity) and/or health plan contract management preferred. Spanish speaking preferred.
