JOB SUMMARY: The Lead Care Coordinator, under the administrative/clinical supervision of the CME Clinical Supervisor is responsible for providing care coordination services to clients and families identified by the CME Catchment Area (CCA). In addition to the responsibilities of Care Coordinator, the Lead is responsible for assisting in the training and providing shadowing experiences for newly hired Care Coordinators within their first six months of employment. The Lead Care Coordinator also coordinates formal and informal services for the child and family in accordance with the principals of the High Fidelity Wraparound model associated with the CME project. This includes, but is not limited to, engaging families in Wraparound model, planning, implementing and monitoring service provision, and collaboration with involved partners.
ESSENTIAL DUTIES: Coordinate with the CME Clinical Supervisor and Director to provide supportive training to new hires in their probation period. Allow new hires to shadow day to day care coordination expectations that include home visits, Child and Family Team meetings, development of crisis and safety planning, care plans and CANS assessments. Provide assistance, as needed, in the interviewing, selection, and onboarding of new hires. Ensure availability to support new hires in the performance of their essential duties during their probationary period in the office. Provide Care Coordinator services as part of the CME Project, using the High Fidelity Wraparound model to clients and families identified for the projects. Deliver service in a variety of settings in the home and community. Service plan should include a comprehensive 24 hour Crisis Plan. Maintain required caseload of 1:10 in the CME Project at any given time. Initial Plan is required within 30 days, and subsequent plans submitted every 30 days. Engage families in the Wraparound process towards development of the Child and Family Team. Facilitate child and family team meetings using High Fidelity Wraparound model. Complete all required assessments and documents as outlined by the agency and the CME Project to include the Strengths, Needs and Cultural Discovery Assessment and the Wraparound plan. Work collaboratively with identified partners on behalf of the Child and Family team to include both formal and informal supports. Provide crisis management as necessary. Monitor the provision and quality of services provided to the family through the Child & Family Team and act as liaison when new services/resources need to be sought or developed. (Essential Function) Arrange for transportation of enrolled children/families to appointments, crisis/respite services, etc., if needed. Contribute to the development and maintenance of the client record through the timely completion of assigned documentation in accordance with applicable licensing and accreditation regulations and standards. Provide written and verbal information related to the youth's and family's mental health based on assessment and family contact. This information will include the youth's and family's strengths and competencies, progress or lack of progress, as well as report on the services and supports put in place to assist the family. Provide a comprehensive safety/crisis plan with innovative strategies to assist the youth in being maintained safely in the least restrictive setting. Respect the privacy of clients and hold in confidence all information obtained during the client's treatment. All client-related documents should be handled in accordance with Agency guidelines on confidential material. Has/Maintains a good on-time and attendance record. Perform other duties as assigned
QUALIFICATIONS :
Education: Minimum Bachelor's degree required with three years of experience in the mental health field. Master's Degree in Social Work, Counseling or related field with one to two years of experience in the mental health field preferred.
Licensure: Valid Ohio LPC or LSW preferred. Skills/Competencies: Strong clinical skills including expertise in systemic family therapy, crisis intervention, family education, and linking/ advocacy skills. Completion of Vroon Vandenburg High Fidelity Wraparound Training. Core Expertise: Possesses skill, knowledge and abilities to perform the essential duties of their role; keeps knowledge up to date. Independent Judgment: Ability to perform job responsibilities with a high degree of initiative and independent judgment. Leadership: Proven effective leadership skills to ensure departmental engagement and success. Cultural Competency: Demonstrates awareness, sensitivity and skills in working professionally with diverse individuals, groups and communities who represent various cultural and personal background and characteristics. Interpersonal Communication: Communicates clearly using verbal, nonverbal, and written skills in a professional context; demonstrates clear understanding and use of professional language. Experience: A minimum of one year experience as an OhioRise Care Coordinator required. Demonstrates full knowledge of the role as well as consistently meets the care coordination expectations. Other: A valid driver's license with approved driving record(less than 6 points), personal transportation and insurance, if required to drive on behalf of the agency.
