Under the direction of Treatment Coordination supervisor, Treatment Coordinators assist people with intellectual and developmental disabilities (I/DD) referred for Center for Specialty Therapy and Premier Healthcare Article 16 Nutrition, Rehabilitation and Mental Health services with securing, maintaining and coordinating access for OPWDD authorized services to maintain continuity of care. Receives referrals, reviews eligibility and conducts on-site, in-person and virtual patient intakes, coordinating and following up with various parties to obtain and maintain documentation and updates needed to support initial and continued eligibility for an assigned caseload. Serves as an advocate for the people we support, regularly liaising with circles of support, external entities and a variety of internal partners including clinical supervisors and support staff, Compliance and Quality, Billing Department, to facilitate and coordinate services or to secure eligibility.

Minimum Qualification Requirements including education, experience, licensure/certification, etc. and essential physical capabilities (e.g. lifting, assisting lifting, standing, etc.)

Bachelor’s degree, preferably in health or social sciences; and

One (1) year of experience working with individuals with intellectual and developmental disabilities (I/DD); or

One (1) year of experience in a patient-facing role in an outpatient clinic, doctor’s office or similar healthcare environment including medical receptionist, medical secretary or similar medical support roles.

Ability to work autonomously and work well in groups, including building relationships and working collaboratively with people at all levels, including internal staff and circles of support.

Excellent interpersonal and customer service skills, including the ability to accurately communicate, collect and present information, both verbally and in writing.

Proficiency with computers and electronic systems, including NextGen or other electronic health record (EHR) and Microsoft Office, specifically Outlook, Word and Excel and the ability to quickly learn to use common office equipment and various electronic systems and software programs (e.g. Workday, Practice Management, OPWDD CHOICES, ePaces, etc.) to complete essential tasks.

Self-directed, highly organized, with good time management, record-keeping and follow-up skills, including the ability to appropriately prioritize multiple tasks to meet prescribed timelines.

Exceptional problem-solving skills, with the ability to identify and resolve concerns or issues to secure eligibility in support of continuity of care.

Ability to report to clinic location in assigned borough of New York City to conduct on-site intakes multiple times per month, as required by caseload and to report to NYC Headquarters at least once per month.

Ability to meet essential physical demands of position, including the ability to sit for extended periods, lift and carry files or documents weighing up to 10 pounds and move around the office.

Preferred Qualification Requirements (desired requirements beyond MQRs above)

Knowledge of OPWDD Article 16 eligibility requirements

Familiarity with medical terminology, specifically disorders, diagnoses and/or treatments applicable to people with I/DD

Proficient in NextGen electronic health record

Verbal fluency in Spanish required

Schedule: Monday to Friday 9am to 5pm

Pay Rate: $22.01 per hour
