Work Arrangement Training:
Conducted on-site in the Bronx Hybrid Schedule:
4 days remote, 1 day field-based (either Manhattan or Bronx area)
We are seeking a compassionate
Registered Nurse
to join a growing
Managed Long-Term Care (MLTC) team . In this role, you will coordinate care, conduct in-home assessments, and support older adults in living safely and independently.
Key Responsibilities Conduct comprehensive in-home assessments and reassessments of members’ health and functional status. Develop and maintain individualized care plans tailored to each member’s needs. Coordinate and authorize covered services across providers and payers. Communicate and collaborate with members, caregivers, and interdisciplinary teams to ensure optimal outcomes. Serve as a clinical advocate, promoting member well-being and continuity of care. Maintain accurate documentation and compliance with MLTC guidelines and policies.
Requirements: Active NYS RN license & BSN Minimum 3 years RN experience; MLTC or geriatrics experience preferred
What We Offer: Salary: $105K–$115K +
