Leasing space inside Walmart stores and providing eye & vision care to customers.
Position Summary As an Independent Optometrist leasing space within Walmart, you will take a leading role in patient care, including eye and vision services as well as general health and well-being.
What You’ll Do
Launch and manage your services in Walmart stores.
Provide patient care for eye and vision services.
Use your autonomy to establish days of practice, hours of operation, patient schedule, and fee schedule.
Keep 100% of exam/professional fees.
Lease Agreement Options
Fair market value equipment/instrumentation.
Administrative and/or Opt-Tech support services (if state allows).
Minimum Qualifications
Doctor of Optometry (OD) from an accredited optometry school.
Valid state optometry license.
Ophthalmologist (MD or DO) from an accredited school.
Valid state ophthalmology license.
General Liability Certificate.
Professional Liability Insurance Certificate.
W-9 form.
