Choose to receive administrative and/or optician technology support services if your state permits

Minimum Qualifications

Doctor of Optometry (OD) from an accredited optometry school

Valid state optometry license

Ophthalmologist MD or DO from an accredited medical school (if applicable)

Valid state ophthalmology license (if applicable)

General liability or professional liability insurance certificate

Additional Information ME/VA license agreements are managed by Gumberg Asset Management Company.
