Job Description:
Evaluates residents prior to move-in to determine whether their care and service needs can be met in the community. Conducts re-evaluations as indicated by the residents condition and/or State regulations.
Develops a service plan unique to each resident based on the evaluation and the residents stated preferences. Revises the service plan as indicated by the residents condition and/or State regulations.
Collaborate with each residents physician, pharmacist, third party healthcare providers, and other members of the healthcare team to promote coordinated care and services.
Participates in the oversight of the communitys medication management program. Collaborates with the pharmacy, conducts med tech training and skill validation, performs routine audits and other activities that promote safety in medication administration.
Maintains a working knowledge of all applicable local, State, and Federal regulations.
Assures that resident evaluations and documentation meet all requirements and collaborates with the HCD to assure that the resident care department is survey ready at all times.
Conducts interviews, orientation, coaching and ongoing training of Harmony Square team members in accordance with associate training standards and regulatory guidelines.
Provide supervision and direction to Harmony Square team members in collaboration with the Healthcare Director to ensure consistent, high-quality care and support.
Collaborate with the Life Enrichment Director to ensure the delivery of purposeful daily programming within the community and through external outings and events.
Lead a minimum of 1 high quality, innovative program per month.
Plan, lead, and facilitate monthly Family Council Meetings to foster open communication, share updates, and strengthen family engagement within the community .
Partner with Alzheimers Association or other local organizations to schedule, organize and participate in Support Group meetings monthly
Utilize and connect with local community resources.
Maintain Behavior Expression process.
Ensure all aspects of dining are being met through partnership with the Director of Dining and Health Care Director.
Completes daily and bi-weekly neighborhood rounds.
Leads and organizes daily huddles.
Non-Essential Functions:

Establish and maintain a positive working relationship with all departments.
Effectively communicate with resident families by responding to questions/concerns promptly.
Complete all required courses in adherence with state regulations and Relias Learning.
Passion for working with individuals with dementia.
Carry out other duties as assigned.
Qualifications/Skills/Educational Requirements:
Current valid and unencumbered license to practice as an LPN in the State in which the community is located.
Area of Study: Nursing. Experience in caring for geriatric individuals and those with Alzheimers and other forms of dementia strongly preferred
1-2 years experience required; 3 or more years' experience preferred.
Current CPR certification required. Dementia training or Certified
Dementia Practitioner preferred or eligible for certification.
Strong organizational and time management skills
Effective verbal and written communication skills.
Ability to work a flexible schedule including day, evening and night shift and weekends.
Ability to be on-call after hours.
Ability to act as the Manager-on-Duty on selected weekends
Experience with Behavior Expressions redirection and interventions
