Independent Optometrist leasing space inside Walmart stores – overview
As an Independent lease holder, you will take a leading role in eye and vision care as well as general health and well-being while providing patient care in Walmart stores. You have autonomy over practice logistics and income, with options for equipment, instrumentation, and support services where allowed by state law.
What you’ll do
Establish your days of practice and hours of operation
Control your patient schedule
Set your fee schedule and keep 100% of exam/professional fees
Opt into equipment/instrumentation and administrative/opt-tech support services where permitted
Minimum Qualifications
Doctor of Optometry (OD) from an accredited optometry school
Valid state optometry license
General Liability Certificate
Professional Liability Insurance Certificate
W-9
ME & VA license agreements are with Gumberg Asset Management Company
