Doctor of Optometry (OD) from an accredited optometry school Valid state optometry license Preferred Qualifications
Ophthalmologist (MD or DO) from an accredited school Valid state ophthalmology license General Liability Certificate Professional Liability Insurance Certificate W-9 Licensing & Compliance
