Under general supervision, the Pharmacist performs professional pharmaceutical duties for the San Francisco Department of Public Health (DPH) to assure quality patient care, appropriate drug therapy and regulatory compliance. Positions are located throughout DPH including San Francisco County Jails, Community Behavioral Health Services, and various clinics and Health Centers located throughout the City and County of San Francisco. Some positions may require work with individuals in locked jails and psychiatric facilities, the homeless population, substance use disorder, or individuals who may be carriers of infectious diseases. Shifts may include evenings, nights, weekends, rotating and/or holidays. Working hours vary by location. Responsibilities

Reviews, processes, compounds, prepares, labels, and dispenses medication orders and prescriptions. Monitors drug therapy for appropriateness, contraindications, interactions, adverse effects, and therapeutic/toxic dosing. Communicates drug information and/or medication counseling to patients and health care personnel. Performs quality assurance by conducting drug use evaluations and medication area inspections. Maintains accurate and thorough documentation of patient care activities and regulatory requirements. Supervises support personnel, as required. In addition to the essential duties listed above, Pharmacist positions in designated areas perform additional duties: ZSFG INPATIENT PHARMACY POSITIONS : Reviews, processes, dispenses physician medication orders and prescriptions including IV admixtures and chemotherapy; monitors and documents clinical interventions in the EHR; provides therapeutic drug monitoring; manages inpatient pharmacy workflow. JAIL HEALTH POSITIONS : Acts as Pharmacist-in-Charge and ensures compliance with pharmacy laws and regulations. BEHAVIORAL HEALTH SERVICES POSITIONS : Reviews and dispenses medication orders for mental health and substance use disorder; assists with insurance navigation; may provide observed dosing and point-of-care tests; participates in chart reviews and inter-disciplinary teams. The 2450 Pharmacist also performs other related duties as assigned. Qualifications

Minimum Qualifications License: Requires possession of a valid Registered Pharmacist license issued by the California State Board of Pharmacy. (Please attach a copy of license) Possible Special Conditions (some positions may require the following): Six months inpatient pharmacy experience within the last five years. One year of pharmacy informatics experience in EHR/PIS and CPOE within the last three years. Bilingual fluency in languages needed by the department; passing bilingual proficiency exam required for bilingual positions. Ability to obtain and maintain security clearance with the San Francisco Sheriff’s Department. Experience in Pharmacy supply chain and inventory management and/or 340B Program. Other Possession of Epic module certification applicable to your Epic specialty area is a condition of employment for certain IT-related positions; certification must be obtained within six months of hire. Applicants must meet minimum qualification requirements by the final filing date unless otherwise noted. One-year full-time employment is equivalent to 2,000 hours. Selection procedures include MQSQ, Training and Experience Evaluation, and possibly additional steps; language proficiency exams may apply for bilingual roles.
