Weekend Requirement: 1 weekend day shift required every 4th weekend... Floating Required: Float to Washington and Stratford campus if needed... Manager is looking for a traveler who doesn't live more than 1.5 hours from the facility... Charting system: Epic... Patient Diagnosis: All medical surgical acute care discharge planning. No behavioral health, no Psych, No counseling components... Experience Required: Experiences with acute care discharge planning or homecare/community discharge planning. This is not a counseling MSW position... Dress Code: Business Casual...
