Behavioral Health Professional , youll support Medicare/Medicaid members through in-home or virtual assessmentshelping improve their independence and quality of life. Provide clinical support and collaborate with the Interdisciplinary Care Team (ICT) for members with medical complexity. Develop, implement, and coordinate individualized care plans to ensure appropriate services and address barriers to care. Conduct medical attestations, clinical reviews, and ensure services are provided in the least restrictive setting. Support members and caregivers in accessing social, housing, educational, and other community resources. Serve as the primary point of contact for the ICT and coordinate with internal and external stakeholders to meet member needs. What Youll Bring: Michigan LBSW or LLBSW (MSW preferred) 2+ years in behavioral health or clinical care management Excellent communication and coordination skills Valid drivers license, reliable car, and home office with internet
