Overview
Valley West Healthcare Center is a long-term care and post-acute care facility in Eugene, Oregon. We provide high-quality nursing and therapy services with strongemployee morale and retention. We are currently looking for a Full-time/Part-time and PRN Speech Language Pathologist to join our team.
Compensation and Benefits
Full-time/Part-time: $40 - $50/hr DOE
PRN: $50 - $55/hr DOE

In-house rehab programs with empowering work environments
Responsibilities
Provide direct rehab care in speech therapy to patients upon physician referral in accordance with applicable laws, regulations, and Life Care standards
Education, Experience, and Licensure Requirements
Graduate of an accredited program in speech therapy (minimum MS or MA SLP)
ASHA certified (CCC)S OR must be eligible for 9-month Clinical Fellowship Year (CFY) to earn ASHA certification; must maintain ASHA credentialing thereafter
Currently registered/licensed in applicable state; active license in good standing throughout employment
One (1) year experience in post-acute care or related setting preferred
CPR certification upon hire or during orientation; CPR certification must remain current during employment
Specific Job Requirements
Participates in community health matters/projects as appropriate
Proficient in Microsoft Word, Excel, and email
Liaises with patients, families, and support departments to plan for patient needs
Demonstrates good body mechanics at all times
Ability to make independent decisions when warranted
Knowledgeable of post-acute care laws, regulations, and guidelines
Implements and interprets department programs, goals, objectives, policies, and procedures
Maintains patient rights, safety, and sanitation; maintains professional relationships
Maintains confidentiality of proprietary information
Adheres to company policies including harassment and compliance procedures
Displays integrity and professionalism per Life Care Code of Conduct and completes required compliance trainings
Essential Functions
Establish, assess, and modify measurable goals
(SLP CCC only) Oversee and evaluate care given by SLP CFYs
(SLP CFY only) Follow treatment plans under supervision and in accordance with federal/state guidelines
Chart appropriately and timely
Utilize therapy software appropriately and accurately
Provide excellent customer service and maintain a positive attitude toward patients
Assist in patient evacuation as needed
Maintain dependable, regular attendance
Exercise good judgment and work effectively on an interdisciplinary team
Read, write, speak, and understand English
