Job title: CCHIP Senior Integrated Care Specialist Department: CORE Center Location: Department of Public Health Shift: 8:00AM – 4:00PM Job type: Exempt; Non-Union; Career Service status note: This position is exempt from Career Service under the CCH Personnel Rules. Grant-funded position: Grant 2025 CCH Ryan White Title IV-PT D until July 31, 2026, with potential renewal. Summary: The CCHIP Senior Integrated Care Specialist will conduct HIV testing, outreach (HIV awareness/lost to care), recruitment, and linkage to care services, providing enhanced care coordination to persons living with/affected by HIV/AIDS as directed. The Senior Specialist will work closely with newly diagnosed HIV clients to ensure linkage to care, patient navigation, and medication adherence at Cook County Health (CCH) Ambulatory & Community Health Network (ACHN) Clinics. The role promotes program enrollment and the use of antiretrovirals (ARV) for HIV treatment and prevention. It oversees retention/case finding activities to locate newly diagnosed HRSA eligible patients, and works with a team of peer educators and clinical staff on community outreach, HIV testing, and health education onsite at ACHN locations and surrounding areas. Note: This is a grant-funded position with potential renewal. Responsibilities
Provide direct oversight of CCHIP Integrated Care Specialists and staff in the absence of the SHCC. Collaborate with leadership to maintain service delivery workflows and engagement of care. Serve as point of contact for CORE primary care patients needing confirmatory testing. Assist with one-to-one training of CCHIP staff across sites working in the Sexual Health Clinic at CORE. Draft and submit operational and productivity reports, including weekly reports on patient intervention and progress. Assist with planning, implementation, and evaluation of health education initiatives and policy/procedure revisions. Monitor, maintain, and restock inventory of supplies, including point-of-care testing kits (e.g., INSTI, Determine). Conduct outreach, recruitment, HIV testing, pre/posttest counseling, and risk reduction counseling for at-risk populations in clinical settings at ACHN locations; include prevention and treatment activities. Conduct case reporting for CORE HRSA eligible patients, partner services, and health outcomes documentation. Facilitate linkage to care, health education/literacy assessments, and specialty care referrals; verify eligibility forRyan White services and related programs. Provide counseling on HIV prevention methods (PrEP, PEP) and assist with enrollment and education on coverage programs. Document services in the EMR (CAREWare) and ensure accurate charting for audits. Participate in weekly case conferences and planning with the care team; collaborate to expand outreach and improve service delivery models for HRSA-eligible patients. Support program planning and evaluation through data collection and reporting across CAREWare and other platforms. Adhere to all applicable state, local, and federal laws; participate in community testing and outreach events as necessary. Minimum Qualifications
Bachelor’s degree from an accredited college or university (official transcript required at interview). Two (2) years of experience in health education, care coordination, HIV/STI prevention, or related social service program delivery. Cultural competence and experience with diverse populations (e.g., MSM, recently released from incarceration, sex workers, low income, substance users, homeless). Ability to work non-traditional hours including nights or weekends as required. Must obtain CDPH/IDPH HIV Testing & Counseling certification within 3 months of starting and be trained/certified in CDPH CHIMS within 6 months. Proficiency with Microsoft Office (Word, Excel). Preferred Qualifications
Bachelor’s degree or higher in a health-related field; additional education preferred. Three or more years of HIV care and support services experience. Experience in hospital, outpatient, or community settings; CAREWare proficiency; HIV Testing and Counseling certification; DIS certification; bilingual abilities. Knowledge, Skills, Abilities
Excellent verbal and written communication; strong interpersonal skills across diverse populations. Customer service orientation, empathy, and professional demeanor. Computer literacy and ability to multi-task in a fast-paced environment. Ability to handle confidential information and maintain professional composure under stress. Physical ability to stand/sit for extended periods and lift up to 20 lb occasionally; frequent walking; occasional bending. Physical and Environmental Demands
