Overview

We are seeking a highly skilled and compassionate Care Management Coordinator to join our outpatient neurology practice. As a Care Management Coordinator, you will support the Care Management Physician and team to ensure that our patients receive the highest quality of care and support throughout their treatment journey. Your primary role will be to coordinate and manage care for patients with neurological disorders, including those with multiple comorbidities. Duties And Responsibilities

Structured recording of patient health information Performs administrative tasks that streamline the onboarding of patients and determines which care management services are warranted, by working with ordering providers and assistants in placing orders for care management services. Receives Chronic Care Management orders from physicians, accesses patient charts, and imports information into Care Management dashboard systems (MemberCare, Prevounce, Allscripts). Documents appropriately in the CCM dashboard and EMR. Enters encounter details and reports at month-end for physicians to sign off. Abides by all HIPAA and confidentiality regulations and policies. Maintaining comprehensive electronic care plans Facilitates and maintains care plans in conjunction with provider and patient. Ensures patient care plan compliance. Managing care transitions and other care management services Assists patients with coordination of appointments and other care management needs related to their chronic conditions. Assists with transitional care management when the patient has been discharged from a hospital. Coordinating and sharing patient health information promptly within and outside the practice Ensures sharing and gathering records with the patients’ primary care physician. Ensures sharing and gathering of records from patients’ other health care providers. Gathers hospital records when applicable. Supports the patient with chronic diseases in achieving health goals Works closely with patients, families, and providers to ensure coordinated and effective care in accordance with care plan. Assists with referral management. Maintains communications with patients via various methods and escalates urgent concerns to the clinic care team (physician/Medical Assistant) via the proper channels. Patient and caregiver engagement Assesses patient and caregiver needs. Educates and guides patients to understand the benefits and requirements of care management services as needed. Assist patients, families and caregivers with accessing community resources. Revenue cycle Learns and understands the requirements for CCM billing. Works with authorization specialist on determining insurance eligibility and authorization requirement for care management support. Maintains communication with patients monthly or as needed to fulfill billing requirements. Able to meet monthly metrics as determined by Care Manager Specialist and management. Provides any other duties and responsibilities as determined by management and physician leadership. Supervisory Responsibilities

High school diploma or equivalent required Medical Assistant Certification Community Health Worker Certification preferred Work experience in an outpatient healthcare facility preferred Customer service work experience in the field of healthcare Physical Requirements

Work is performed primarily in an outpatient clinic environment. Routine walking, climbing, lifting. Work may require sitting for long periods of time while staying fully alert. Work will require rooming patients, setting up/taking down exam rooms and working alongside other healthcare professionals in a clinical setting.
