The Day Program Coordinator is responsible for overseeing the development and implementation of the Day Habilitation Service Plan. The Day Program Coordinator is responsible for integrating the Day Service Plan and activity schedule for the individuals. The Coordinator will act as the liaison between Care Manager, community and Day Services Program. Essential Job Functions
Review the Day Service Plan for each individual, assuring it is consistent with the 3 IPs (independence, integration, individuality, productivity) and it includes input from the individual, family advocate and case manager. Ensure that all necessary assessments and evaluations needed for the completion of the Day Service Plan are complete. Coordinate delivery of services with residential program; parents of those living at home. Assist in the development of individual team productivity, allocate resources and staff. Assist the Program Manager in fire drills and emergency evacuation drills. Assist in obtaining all services outlined in the Day Hab Plan with the individual and advocate. Review monthly reports on progress toward goals, outcomes, behavioral and medical issues for all participating individuals. Match Day Hab plans to Life Plans. Develop goals for the individual, ensure progress is tracked, review progress daily, and trend data on a monthly basis, documenting outcomes and making appropriate recommendations. Attend all Individualized Service Plan meetings and applicable reviews. Be alert to individual medical concerns, illnesses and accidents. Report promptly and follow up as necessary. If there are changes in Day Service Plan, write addenda for the Life Plan and forward to Case Manager for sign-off and review. Schedule Semi-Annual and Annual reviews in conjunction with Care Managers. Maintain contact with Care Managers regarding individuals\' medical and behavioral concerns and obtain required documentation (Life Plan and clinical summary). Ensure the Day Habilitation Program is adequately staffed, based on regulatory ratios. Provide supervision, good role modeling and crisis intervention as needed. Provide direct supervision and guidance to the Instructors and Assistants. Train Direct Care Staff as required. Establish rapport with community members, fellow staff and the individuals. Responsible for handling all issues with the parents of the individuals assigned and ensure issues are communicated with management. Assist the management of the Day Hab program as required. Must comply with all other duties as assigned. Qualifications
Bachelor’s in Human Services, or related field preferred Minimum of 2 years of experience in human services required, preferably in OPWDD setting Knowledge of 624 and 633 also preferable Demonstrated ability to implement and manage change 1-2 years supervisory experience preferred Ability to lead, motivate, supervise, evaluate, coach, train and manage people Strong written and oral communication skills Demonstrated ability to collaborate and work cross-functionally Hold and upkeep certifications in CPR and SCIP Seniority level
