As a Clinical Nurse Navigator, you will serve as a vital link in coordinating care for children diagnosed with sickle cell disease. Your role involves working closely with patients, families, and a multidisciplinary team of healthcare professionals to develop personalized care plans that address medical, emotional, and social needs. You will ensure timely access to tests, treatments, and follow-up care, while providing education and support to empower families. Responsibilities
Coordinating comprehensive care during clinic visits, ensuring patients see all necessary providers. Managing patient education, medication refills, FMLA documentation, school notes, and scheduling return visits. Keeping local, state, and national databases up-to-date. Triaging sick calls, assessing patient needs, and coordinating urgent care or same-day appointments with clinic providers. Monitoring lab results, responding to messages in Cerner, and supporting overall care coordination efforts. This role requires a team-oriented, organized, and proactive individual with excellent communication skills and a compassionate approach to patient care. Schedule

Associate Degree in Nursing (ASN) required; Bachelor of Nursing (BSN) preferred Active Registered Nurse (RN) license in Indiana or via Nurse Licensure Compact (NLC) Graduation from a nationally accredited nursing program 3-5 years of clinical experience preferred, ideally within pediatrics, hematology/oncology, or care coordination Basic Life Support (BLS) certification through the American Heart Association (AHA) required Additional advanced life support certifications may be required based on department policies Knowledge of medical terminology, electronic health records, and database management Strong computer skills, including experience with various digital platforms Prior experience working with pediatric patients is preferred, but not required.
