Overview
Malignant Hematology Clinical Pharmacist
SUNY Upstate Medical University is currently seeking a Malignant Hematology Clinical Pharmacist to join the Pharmacy-DT department. This clinical pharmacist will primarily provide pharmaceutical care for adult, malignant hematology patients at the Upstate Cancer Center (DT Campus). This will include playing a lead role in the management of all pharmacy aspects of Cellular Therapies (CT) and CAR-T (Chimeric Antigen Receptor T-cell) therapy program maintenance. This individual will be responsible for establishing relationships with the adult malignant hematology providers and service representatives.
Responsibilities
Daily patient care as part of an outpatient multi-disciplinary team, building oncology treatment plans, and maintenance of policies/procedures related to malignant hematology and oncology services.
Formulary management and patient counseling.
Based on operational needs, contribute inpatient malignant hematology / BMT service activities to maintain continuity of care.
Interpret medication orders and prescriptions to ensure that they are complete, safe, rational, and compatible with institutional policy.
Prepare and dispense medications for inpatient and ambulatory care use, including injectable sterile products, and dispense pharmacy-related items.
Perform duties in all areas of Pharmacy to include inpatient, ambulatory care, and satellites.
Provide information to health care practitioners to promote the rational use of medications; provide drug monitoring services; maintain pharmacy records by law and institutional policy.
Interpret policy and procedures for hospital staff; participate in teaching programs for hospital staff; participate in Pharmacy Department quality assurance programs.
Qualifications B.S. in Pharmacy or Pharm.D and NYS Pharmacist license and registration (or eligible) required.
