Position: Independent Optometrist leasing space inside Sam’s Club.
Employer: Eyes On Eyecare.
Salary: Based on experience.

Key Responsibilities:
Establish days of practice and hours of operation.
Control patient schedule and set fee schedule.
Maintain 100% of exam/professional fees.
Fair Market Value Options (optional):
Advanced equipment/instrumentation.
Administrative and/or opt-tech support services if state allows.
Minimum Qualifications for Lease Agreement
Doctor of Optometry (OD) from an accredited optometry school.
Valid state optometry license.
Ophthalmologist MD or DO from an accredited school.
Valid state ophthalmology license.
General liability certificate / professional liability insurance certificate.
Additional Information: Lease agreements are handled by Gumberg Asset Management Company.
