Travel/Strike Assignment Position Summary The RN Case Manager supports the physician and interdisciplinary team in coordinating patient care, discharge planning, and utilization management to ensure efficient care delivery and high-quality outcomes. The role requires strong clinical knowledge, patient advocacy, and the ability to navigate complex hospital systems. Key Responsibilities Coordinate and facilitate discharge planning and transitions of care Obtain and manage payor authorizations and precertifications Collaborate with the care team to remove barriers to timely care and reimbursement Support clinical documentation improvement and communication with coders Serve as a liaison between physicians, patients, families, and payors ✅
Requirements Education:
BSN required License:
Active RN license in Michigan (required) Certifications:
BLS (AHA required) Experience:
Minimum 5 years acute inpatient case management experience Experience with discharge planning and utilization management Experience using CarePort system required Familiarity with AllScripts, GEMS/Sunrise preferred
Must be comfortable working on the floor with patients and able to acclimate quickly Schedule Shifts:
