Supplemental Questionnaire

A Supplemental Questionnaire consists of a series of questions and a mechanism for obtaining an applicant's qualifications and experience in job-related areas. 1. The Solutions Consulting Services (SCS) Unit is not a traditional IT manager's office - it is a consulting practice within CalHHS. Describe a time when you had to design or build a new service, program, or business function from the ground up (not just manage something that already existed).

• What problem or need drove the creation of that function? • How did you design the service model (intake, delivery, measurement, improvement)? • What challenges did you encounter and how did you address them? • What outcomes were achieved and how did you measure success?

2. As Manager of the SCS Unit, you will often serve as both advisor and leader, balancing the needs of executives, program staff, and technical teams. Think of a time when you were in a consulting or advisory role where a client or stakeholder wanted a solution that you knew was not the best option.

• How did you navigate the situation? • What approach did you take to influence decision-makers? • How did you balance diplomacy with ensuring the right outcome? • What was the final result?

Additional Information

This position is designated as required to file a Statement of Economic Interest (Form 700) under the California Health and Human Services Agency Conflict of Interest Code. The position is responsible for the making, participating in the making, or using his or her official position to influence the making of governmental decisions that may potentially have a material effect on his or her personal financial interests. The appointee is required to complete a Form 700 within 30 days after his or her appointment. Failure to comply with the Conflict of Interest Code requirements may void the appointment.
