This role assumes responsibility for the medication management of assigned patients; it includes medication reconciliation, medication monitoring, patient education, and evaluation of medication therapy. The pharmacist remains involved in education of Pharmacy and Medical Residents and as needed, provides education to health care providers. The pharmacist also provides expert content for order sets, benefit design, formularies, and other pharmacy related documents.

Responsibilities

Provide pharmacokinetic consultation, obtain and use appropriate laboratory data to evaluate drug medical selection and dosing and communicate findings to health care practitioners.

Evaluate patient response to drug therapy.

Document interventions and conduct outcome studies/evaluations to exhibit good patient outcomes.

Assist with the development of pathways, best practices, and guidelines for the assigned population.

Educate staff, students, patients, health care providers and other health care professionals; submit for publication once every two years.

Mentor residents, students and new staff to meet departmental goals.

Ensure overall patient care beyond the hospital visit, through all transitions of care.

Volunteer for work/task during staffing shortages due to inclement weather, power outages, computer downtime or other situations.

Adapt communication to the specific patient population, including privacy, introduction style, service explanations, and obtaining permissions.

Perform any related duties as required.

Education

Required: Bachelor of Science in Pharmacy.

Completion of a PGY1 Pharmacy Practice Residency (3 years related specialty experience accepted in lieu of PGY1 Residency).

Preferred

Doctor of Pharmacy (PharmD).

Completion of a PGY2 Specialty Pharmacy Residency.

Work Experience

Required: None.

Certifications

Required: Current Pharmacy license in the state of practice.

Knowledge Skills And Abilities (KSAs)

Must have computer skills and dexterity required for data entry and retrieval of patient information; proficiency with Windows-style applications and keyboard.

Effective verbal and written communication skills and ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.

Strong leadership/teaching skills.

Physical and Environmental Demands Medium Work: Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. Must be able to stand for prolonged periods of time, travel throughout and between facilities, and routinely work with exposure to blood, body fluid, and tissue. The incumbent may work in isolation rooms and has occupational risk for exposure to hazardous medications and waste.
