Duties
Effectively screen/evaluate patients within short‑term and long‑term rehab and opportunity for outpatient.
Utilize standardized assessments and evidence‑based practice to support clinical interventions.
Provide rehabilitative, skilled, and medically necessary treatment intervention to patients within short and long‑term settings.
Administer a wide variety of interventions as part of the plan of care based upon individual patient needs.
Assess patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
Provide consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
Effectively communicate with the OneClinical Team regarding patient progress and barriers.
Provide comprehensive discharge summary of services provided per regulatory and clinical requirements.
Qualifications
Licensed Speech‑Language Pathologist.
Open to SLPs at all experience levels.
