Job Summary:
This part‑time/per diem role provides per visit pay and involves overseeing clinical care and treatment of hospice patients and their families. The NP consults with attending physicians and hospice physicians, directs the Interdisciplinary Group/Team, and ensures quality care.
Responsibilities
Collaborates with the Interdisciplinary Group to assure appropriate evaluation and palliative treatment of terminal disease.
Consults and assists in developing patient/family care plans to ensure quality care.
Educates patients/families about disease process, self‑care techniques, and end‑of‑life care.
Provides training to staff as needed.
Performs and documents visits based on the plan of care and patient/family need.
Interacts with attending and consulting physicians regarding palliative care.
Provides education to IDG/T members on a PRN basis for clinical palliation issues.
Actively participates in the IDG/T.
Communicates effectively, timely, and appropriately at all times.
Informs physicians and personnel of changes in patient needs and outcomes of interventions.
Completes face‑to‑face encounters with patients, as assigned, prior to the 3rd benefit period and each subsequent period.
Provides clinical assessment results to hospice physician after face‑to‑face encounters.
Uses standard precautions and follows hospice policy.
Adheres to hospice policies regarding protected health information.
Qualifications > Graduate from a Baccalaureate School of Nursing, and hold an NP‑focused Master’s or Doctoral Nursing Program.
Current state license as a Registered Nurse and Advanced Practice Nurse.
Minimum two years’ experience as an Advanced Practice Nurse; hospice or home health preferred.
Proficiency in pharmacology, palliative care, and end‑of‑life practices; good interpersonal skills.
Reliable transportation, valid auto liability insurance, and driver’s license.
