Job Summary MedStar Health is currently seeking a Population Health Dietitian to join its Food RX program at MedStar Good Samaritan Hospital. This position requires 2-3 days working at the Franciscan Center, an offsite community partner. The Food Rx Program is a national award‑winning, regionally based program that helps individuals in the community with chronic diseases and food insecurity. The program provides produce prescriptions and medically tailored groceries designed to help people with diabetes and other chronic diseases live healthier lifestyles. As a Dietitian, you will provide nutrition counseling, diabetes education, weight management strategies, and meal planning. Study participants have shown reduced hemoglobin A1C levels, fewer ED visits, and improved glucose control.
Primary Duties
Work closely with Community Health Advocates at MedStar Good Samaritan Hospital to address the social needs of Food Rx patients. On site Wednesday, Thursday, and Friday.
Collaborate with providers in the Collaborative Care Program outpatient clinic as part of the interdisciplinary team to identify eligible patients, coordinate enrollment and follow-up, and provide MNT/DSME consults. Act as Lead RD for the food pantry at The Franciscan Center in Baltimore, MD.
On site Mondays and Tuesdays. Provide diabetes education, nutrition counseling, weight management strategies, and meal planning to patients within the Food Rx Program.
Work with and report to the Regional Clinical Manager for Food Rx.
Incorporate evidence‑based strategies into the population health management of patients with complex medical and social needs, especially patient populations with food insecurity and uncontrolled diabetes.
Other responsibilities, as assigned.
Qualifications
Bachelor’s degree in nutrition/dietetics required.
Master's degree required if credentialed after 12/31/2023.
Registered Dietitian in state of Maryland required, or eligible to obtain MD licensure required.
CDCES certification preferred.
CDR registration required.
This position has a hiring range of
USD $61,838.00 – USD $111,259.00 per year.
Location: Baltimore, MD.
