Job Summary Primary functions are to assist in overseeing the clinical care and treatment of patients and families; to consult with the patients’ attending physician, in consultation with the hospice physician; provide clinical direction to the Interdisciplinary Group/Team to ensure quality care.

Responsibilities

Collaborate with the IDG/T to assure appropriate evaluation and palliative treatment of terminal disease.

Consult and assist in developing patient/family plan of care to assure quality care is provided.

Consult and educate the patient/family regarding disease process, self‑care techniques, and end‑of‑life care.

Provide training to other staff as needed.

Perform and document visits to patient/family based on plan of care and patient/family need.

Interact with attending and consulting physicians as necessary regarding palliative care, collaborating with hospice physician.

Consult and provide education to other IDG/T members on a PRN basis for clinical issues relating to palliation.

Actively participate in the IDG/T.

Communicate effectively, timely, and appropriately at all times.

Inform physicians and other personnel of changes in the patient’s needs and outcomes of interventions, while evaluating patient/family response to care.

Complete face‑to‑face encounters with patients, as assigned, prior to the 3rd benefit period and each period thereafter.

Provide clinical assessment results to hospice physician after making face‑to‑face encounters with patients, as assigned.

Always use standard precautions, and adhere to hospice policy.

Adhere to hospice policies regarding protected health information.

Qualifications

Graduate from a baccalaureate school of nursing or a nurse practitioner focused master’s or doctoral nursing program.

Current state license as a registered nurse and as an advanced practice nurse.

Two years’ experience as an advanced practice nurse, preferably in a clinical care setting; hospice or home health experience preferred.

Comprehensive knowledge of pharmacology, palliative care, and end‑of‑life care; good interpersonal skills.

Reliable transportation, valid auto‑liability insurance, and a valid driver’s license.
