Position Summary: As an Independent Optometrist leasing space inside our stores you will take a leading role in patient care with respect to eye and vision care as well as general health and wellbeing.
What You’ll Do
Launch your services in Sam's Club.
Provide patient care for eye and vision services and general health and wellbeing.
Establish your days of practice, hours of operation, patient schedule, and fee schedule.
Keep 100% of examination and professional fees.
Opt-In Benefits
Advanced equipment/instrumentation.
Administrative and/or Opt-Tech support services (if state allows).
Lease Agreement Qualifications
Doctor of Optometry (OD) from an accredited optometry school.
Valid state optometry license.
Ophthalmologist MD or DO from an accredited school.
Valid state ophthalmology license.
General Liability Certificate / Professional Liability Insurance Certificate.
ME/VA license agreements are with Gumberg Asset Management Company.
