Community of Hope is seeking a Clinical Nurse Manager to ensure and maintain high‑quality, team‑based patient‑centered clinical care while following COH clinical protocols. This position is located at our Conway Health and Resource Center in SW, Washington, DC.

What You’ll Do
Ensure high quality of care, smooth patient flow, and team coordination for patient visits while following the guidelines of the Patient‑Centered Medical Home model and/or Birth Center model.
Provide direct supervision of Medical Assistant I and Medical Assistant II and assist with coordinating Nurse and Medical Assistant Interns, ensuring proper training of both.
Serve as a liaison between nurses, providers and other office staff.
Prepare patients for examination and treatment, including taking the patient’s history, obtaining accurate vital signs, and verifying the chief complaint.
Assist with clinical procedures performed by the providers in the office as needed.
Assist with Chronic Care Management by providing care and education to patients, including conducting pre‑visit chart reviews as necessary.
Maintain a Pap log to ensure appropriate follow‑up from abnormal Pap results within the designated time.
Monitor outstanding lab reports in eCW to ensure results are received in a timely manner and that patients return to the clinic when a lab is incomplete.
Maintain a Newborn Metabolic Screen log to ensure results are obtained in a timely manner for all newborn patients.
Monitor the functioning of the Vaccine for Children program at the clinic according to CDC and DC regulations.
Meet with supervisees regularly and provide real‑time feedback, training, support, and coaching as needed. Create meeting agendas, send agendas in advance when possible, and document concerns promptly.
Review the performance of team members in a timely manner, completing 90‑day and annual performance evaluations, documenting any issues or changes, and recognizing staff when appropriate.
Implement controlled substance regulations as necessary (for FHBC and Conway only).
Must‑Haves
A current, unencumbered DC Registered Nurse license.
Valid BLS Certificate.
One year of supervisory experience.
One year working in an ambulatory, primary‑care, or med/surg setting.
Knowledge of chronic disease processes and health maintenance.
Ability to work with computers and electronic health records.
Strong verbal and written communication skills.
Maintain current certification as required by law, including CPR or NRP depending on clinic location.
Proof of vaccinations. COH will consider reasonable accommodations for anyone who cannot be vaccinated for a religious or medical reason.
Nice‑to‑Haves
Bilingual in Spanish and/or Amharic.
Prior care management and patient education experience.
Experience working with patients with substance use disorders and behavioral health concerns.
