Clinical Case Manager - CONREP sex offender program

Liberty Healthcare Corporation is accepting applications for a new Clinical Case Manager position with the California Conditional Release Program (CONREP).
This program exclusively serves adult sex offenders who are living in communities throughout California – more specifically, those individuals designated as sexually violent predators (SVPs).
While emphasizing community safety, the program seeks to provide clients with the opportunity for healthy and productive lives in the community:
Program objectives include:
Maintaining community safety by preventing client recidivism
Transitioning clients from hospital treatment into the community
Securing community-based housing
Proactively detecting and addressing client forensic risk factors
Providing behavioral health services, case management, 24/7 monitoring, and transporting clients
As Liberty’s Clinical Case Manager, you will assist the program’s Clinical Director in the oversight of client clinical assessment and treatment services, clinical programming, and community service and supports coordination.
Specific responsibilities will include:
Tracking client progress on community reintegration goals (e.g. employment, budgeting, use of life support funds, obtaining driver's license)
Identifying and addressing barriers to successful client reintegration in the community
Securing community-based services and supports for clients
Coordinating a wide range of client discharge planning activities
Tracking, documenting and reporting on client developments and program processes
Ensuring the timely completion and quality of client assessments, treatment plans, and other client services
Assisting in the oversight of evaluations of clients at the Coalinga State Hospital (travel to CSH is infrequent)
Providing crisis management, assessment and treatment services to clients as necessary
Collaborating with community providers, social service agencies, Department of State Hospital representatives, and various other stakeholders
Providing back up coverage for the program’s Clinical Director and participating in the program’s management on-call rotation
This is a remote first position which will primarily allow you to work from your home office in California with occasional travel within California.
As a full-time employee of Liberty Healthcare in this role, you can expect:
Starting salary between $90,000 and $105,000 (depending on qualifications)

You are a good fit for this role if you possess the following:
Professional licensure (or eligibility for licensure) in California as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC)
Assessment, counseling, and case management experience
Experience identifying and securing community-based supports and services
Experience providing services to justice-involved individuals (especially adult sex offenders or other forensic clients) is preferred
Residency in California
