As an Inpatient Psychiatry Clinical Pharmacist Specialist, you will play a vital role in delivering top-tier pharmaceutical care for our adult and geriatric psychiatry units. Your responsibilities will include: Engaging in daily multidisciplinary care team rounds to provide clinical pharmacotherapy services. Monitoring drug therapy to ensure safety and efficacy through established criteria. Conducting medication use consultations with thorough documentation. Delivering personalized education and counseling to patients and their families. Demonstrating your clinical expertise and contributions to the healthcare team. Promoting medication safety in your specialty area. Assessing and refining clinical pharmacy programs for continuous improvement. Evaluating medication use and implementing measurable enhancements within the medication use process. Participating in the maintenance of the Drug Formulary and guiding decisions for the P&T Committee. Leading staff development efforts and serving as a preceptor for pharmacy residents and students. Minimum Qualifications: Graduation from an accredited college/university with a B.S. in Pharmacy or Pharm.D. degree. Board Certification is required (or willingness to obtain within two years). Specialty certification is desirable. Completion of an ASHP-accredited residency is preferred. Licensure:
Must hold a current license in good standing from the Massachusetts Board of Registration in Pharmacy or be eligible for licensure in MA. Candidates must acquire MA licensure within 1000 hours of employment at CHA. Work Experience: Completion of ASHP-accredited PGY1 and PGY2 residencies or equivalent experience. Completion of ASHP-accredited PGY1 residency with a minimum of two years as a clinical pharmacist. Alternatively, five years of pharmacy experience with at least three years as a clinical pharmacist.
