Description : TITLE:
Speech and Language Pathologists (SLP) - Long Term Care Position Overview: Speech therapists develop treatment plans to improve swallowing, cognitive and communication abilities, and to decrease aspirations. Communication treatment plans may include techniques that not only help the individual to be better understood, but also to help them better understand others. Leads, guides and directs the delivery of speech language pathology and audiology services in the facility in accordance with local, state and federal regulations, standards and established facility policies and procedures to provide appropriate care and services to residents.
EDUCATION/EXPERIENCE/TRAINING Required
: 2 years SLP experience - Required TX License or compact - Required DUTIES AND RESPONSIBILITIES Evaluate residents to identify speech, language, cognitive-communication, and swallowing disorders. Conduct standardized tests and bedside evaluations to determine the nature and severity of impairments. Review medical histories and collaborate with physicians, nurses, and other therapists to understand each resident’s overall condition. Develop individualized
treatment plans
based on evaluation results and patient goals. Set measurable, functional goals to improve communication, cognition, and swallowing safety. Adjust therapy plans as residents progress or medical conditions change. Provide
speech and language therapy
to improve articulation, voice, fluency, and comprehension. Deliver
swallowing therapy
to enhance oral-motor function and reduce aspiration risks. Use evidence-based approaches to treat
cognitive-communication deficits
(e.g., memory, attention, problem-solving). Implement
augmentative and alternative communication (AAC)
strategies when appropriate. Maintain accurate and timely clinical documentation, including evaluations, progress notes, and discharge summaries. Track resident outcomes and modify interventions based on progress. Ensure all documentation meets
state, federal, and facility
standards.
