The Physical Therapist is responsible for coordinating and providing restorative and rehabilitative Physical Therapy services to participants at the Center and at home, working in close coordination with the PACE Physician, rehabilitation staff, and other interdisciplinary team (IDT) members to maximize participant independence and safety and develop or restore function. The Physical Therapist collaborates with the Occupational Therapist to efficiently operate the rehabilitation department. The Physical Therapist is a member of the management team and facilitates collaborative relationships across the organization and with external stakeholders to promote the values, team culture, and mission. Duties and Responsibilities

Conduct Physical Therapy assessments to determine the Physical Therapy care needs, preferences, and goals of the participants and actively participate in IDT meetings to develop participant care plans. Deliver and document Physical Therapy interventions in accordance with physician orders and/or as agreed upon in the participants’ care plans, including but not limited to providing active and passive exercise, muscle re-education, gait and functional training of ADLs and treatments such as massage, heat, sound, water, light, or electricity. Supervise the Therapy Assistants, including providing training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP). Provide training and instructions to staff, participants, and caregivers/families in the use and care of therapeutic equipment. Order, track, and manage maintenance for equipment. Remain alert to pertinent input from other team members, participants, and caregivers and update the IDT promptly of any changes in participants’ condition or functional status. Work with the other members of the IDT to support smooth care transitions between settings (e.g., hospitals, skilled nursing facilities, home, etc.). In coordination with the Marketing Team, support enrollment of prospective participants into the program. Participate in end-of-life care coordination and support. Manage direct reports, including training, supervising, and mentoring. Be legally authorized to practice in the position location, required. Only act within the scope of his or her authority to practice. Complete all required documentation in a timely and accurate manner. Protect privacy and maintain confidentiality of all company procedures and information about team members, participants, and families. Follow company policies and procedures and participate in any required Quality Improvement activities, staff training, and meetings. Communicate regularly with Supervisor and team regarding workload and priorities. Timely completion of all mandated trainings and education. Timely completion of all mandated occupational health screenings as needed. Exercise flexibility in performing assignments as business needs evolve. Other duties as assigned. Required Skills & Experience

Five years of progressively responsible PT experience. Minimum of two years’ Physical Therapy experience in a clinical setting with a frail or elderly population. Physical Therapy knowledge and skills necessary to treat frail, elderly participants with complex needs. Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations. Two years’ supervisory experience with demonstrated ability to mentor and develop team members. Ability to process accurate information in a timely manner. Excellent leadership, organizational, and communication skills in settings with seniors, their families, and interdisciplinary team members. Experience leading in a data-driven organization, leveraging reports and data to prioritize and manage people and projects. Highly motivated, inspirational leader who is able to innovatively engage, support, and remove barriers for the team to ensure the team consistently meets established goals. Excellent clinical, organizational, and communication skills in settings with seniors, their families, and interdisciplinary team members. Ability to work independently with minimal supervision. Demonstrated ability to prioritize in a fast-paced environment. Experience and competency working with people from diverse backgrounds and cultures. Required Education: Bachelor’s Degree in Physical Therapy. The qualifying Physical Therapy Degree earned outside of the US must be evaluated to the US equivalent to a Degree in Physical Therapy. Preferred Education: Master’s Degree in Physical Therapy or Doctorate in Physical Therapy. The qualifying Physical Therapy Degree earned outside of the US must be evaluated to the US equivalent to a Degree in Physical Therapy. Required Certifications & Licensure

Valid California Physical Therapy License BLS Pay Range

Pay range: USD 66/hr - USD 99/hr (including benefits such as medical, dental, vision, and 401K). Start Date
