ShareSTAFF is seeking a Nurse Practitioner Wound Care for a job in Sacramento, California.

Key Responsibilities Clinical Care Conduct comprehensive wound assessments, including history, physical exam, wound measurement, staging, and documentation.
Develop and implement individualized treatment plans for patients with diabetic ulcers, venous stasis ulcers, pressure injuries, surgical wounds, traumatic wounds, and atypical ulcers.
Perform advanced wound care procedures such as :
Sharp / excisional debridement
Enzymatic and autolytic debridement
Negative pressure wound therapy (NPWT) initiation and management
Compression therapy
Application of skin substitutes and biologic grafts
Use of advanced dressings and topical agents
Order and interpret appropriate laboratory tests, imaging, and vascular studies (e.g., ABI / TBI, venous duplex, x-rays).
Evaluate and monitor patient progress, modifying treatment plans as needed.
Coordinate and communicate care with primary care providers, surgeons, podiatrists, home health, and rehabilitation teams.
Documentation & Compliance Accurately document assessments, procedures, and care plans in the electronic medical record (EMR) in compliance with CMS and Medicare guidelines.
Ensure appropriate coding and billing practices for wound care encounters (CPT / HCPCS).
Maintain accurate and timely wound photographs, measurements, and treatment logs for continuity of care.
Adhere to HIPAA, OSHA, and infection control standards.
Education & Leadership Provide patient and family education on wound care management, nutrition, pressure offloading, and infection prevention.
Educate nursing and ancillary staff on wound prevention, dressing selection, and evidence-based wound management.
Participate in quality improvement initiatives to enhance wound care outcomes and patient satisfaction.
Serve as a clinical resource and mentor to other healthcare professionals within the organization.
Collaboration Participate in interdisciplinary team meetings and case conferences.
Work collaboratively with physicians, nurses, physical therapists, and dietitians to coordinate holistic patient care.
Communicate effectively with referring providers and care facilities to ensure seamless transitions of care.
Qualifications Required :
Master’s or Doctorate degree in Nursing (MSN, DNP, or equivalent).
Current and unrestricted California Nurse Practitioner (NP) license.
National certification as an NP (e.g., ANCC, AANP).
Current DEA license (if prescribing medications).
Current BLS / CPR certification (ACLS preferred).
Preferably but not required, clinical experience in wound care, dermatology, podiatry, or surgical settings.
Preferred Wound Care Certification (WCC, WOCN, CWCN, or equivalent).
Experience with advanced wound modalities such as NPWT, skin grafts, and biologic dressings.
Familiarity with EMR systems and wound documentation software.
Knowledge of Medicare coverage criteria, HCPCS coding, and wound‑care billing practices.
Core Competencies Strong clinical judgment and critical thinking skills.
Excellent communication and interdisciplinary collaboration.
Compassionate and patient‑centered approach to care.
Proficiency in wound measurement, staging, and photography.
