Primary Care Physician (PCP) Responsibilities

Accountability for outcomes with a panel of up to 450 patients, ensuring strong clinical care and cost-effectiveness.

Act as a health coach, helping patients set short- and long-term health goals and following up to improve health outcomes.

Simplify and prioritize care to make behavior change actionable for both patients and care teams.

We are an outcomes-focused, value-based organization. For each PCP’s panel, metrics such as patient admissions per thousand, appointment slot utilization (18–21 slots per day), CGCAHPS, clinical gaps closures, and medical cost measures are regularly measured. Each PCP will have goals for these metrics and is expected to work toward them with center and market leadership and care teams. Culture is vital; PCPs are expected to champion a positive culture of love, accountability, and passion.

Essential Job Duties/Responsibilities

Provide independent care for patients with acute and chronic illnesses encountered in older adults.

Take full accountability for patient care and outcomes; seek consultation from specialists when needed while remaining involved.

Engage with the hospitalist for any patient in the hospital, regardless of hospitalist affiliation.

Conduct assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute or complex chronic health needs.

Lead a care team of medical assistants, care facilitators, and care coordinators for office-eligible patients.

For patients unable to come to the office—hospital, SNF, LTC, or homebound—engage with transitional care teams, case managers, and other resources as available.

Actively manage the center, cover for other providers as needed, and assist in recruiting patients and additional providers.

Perform other duties as assigned and modified at the manager’s discretion.

Education And Experience Criteria

MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty required.

Must be able to obtain or already hold an active State Medical License for the state(s) in which working.

Board certification or board eligibility in a relevant specialty preferred; maintenance through MOC, CME, or retesting required.

Current DEA number for schedule II–V controlled substances.

Basic Life Support (BLS) certification from the American Heart Association.

Pay Range $214,700 – $306,714 Salary
