Position Overview: Sprout Therapy PDX is seeking a compassionate, detail‑oriented Psychiatric Prescriber (PMHNP, NP, MD, or DO) to join our multidisciplinary outpatient therapy practice. The prescriber will provide psychiatric evaluations, medication management, and consultation in collaboration with our therapy and case management teams.
This role is designed as a .6 FTE position (approximately 10–12 patient‑facing hours per week across 2–3 days) with additional time for documentation, coordination, and supervision meetings. Once onboarded, there is room to grow to a full‑time position, depending on clinic need. Care may be provided on‑site or via telehealth within Oregon.

Schedule / FTE
.6 FTE (approximately 10–12 patient‑facing hours per week across 2–3 days)
Approximately 12–14 direct patient hours per week with additional documentation and coordination time
Average 30–35 visits per week (mix of 45–60‑minute intakes and 25–30‑minute follow‑ups)
Compensation and Benefits
60% of collections, with expected annual take‑home between $125,000 – $145,000, depending on caseload and payer mix.
Equivalent to approximately $80–$95 per patient‑facing hour for a .6 FTE schedule.

Key Responsibilities
Conduct comprehensive psychiatric evaluations and medication assessments, including review of medical/psychiatric history, current medications, lab results, risk assessment, and treatment planning.
Develop, prescribe, monitor, adjust, and discontinue psychotropic medications as clinically indicated, in alignment with evidence‑based guidelines and practice standards.
Collaborate with therapists, case managers/QMHAs, intake coordinators, peer support specialists, and other staff to ensure integrated, coordinated care.
Provide medication management services including follow‑up visits, side‑effect monitoring, adherence support, and addressing co‑morbid medical or substance‑use conditions.
Participate in team case conferences, review treatment plans, revise care strategy in collaboration with multidisciplinary team.
Provide consultation and guidance to staff on psychopharmacology, medication interactions, side‑effect management, and client safety (including suicide and risk protocols).
Maintain accurate and timely clinical documentation in the EHR (intake notes, ongoing follow‑ups, medication orders, progress toward goals, side effects/adverse events).
Ensure compliance with state licensure, prescribing regulations (including controlled substances), Medicaid/Medicare billing rules, and agency policies (including confidentiality/HIPAA).
Engage in continuous professional development, staying current on psychopharmacology, behavioral health trends, culturally responsive care, and intersectionality in mental health.
Promote Sprout’s values of clinical excellence, enthusiasm for mental health, intersectional awareness, emotional intelligence, and collaborative problem‑solving across the practice.

Qualifications and Skills Required:
Valid and active Oregon medical license (MD or DO) or nurse practitioner license with prescriptive authority (PMHNP) in Oregon.
Board certification in psychiatry (for MD/DO) or relevant certification/licensure for PMHNP.
Experience in outpatient behavioral health settings, medication management, and working collaboratively with psychotherapy teams.
Strong commitment to culturally responsive, trauma‑informed care and understanding of social determinants of health and intersectionality.
Excellent communication skills (verbal and written), ability to collaborate across disciplines, and comfort working independently in a flexible environment.
Proficiency with EHR systems, documentation best practices, and tele‑health modalities (if applicable).
Preferred:
Experience with Medicaid/OHP billing, prior authorization processes, and utilization review.
Bilingual or bicultural competence (Spanish preferred).
Experience in integrated behavioral health/primary care settings or managing co‑occurring substance use and medical conditions.
Physical Requirements
Prolonged periods of sitting, computer use, and/or client meetings.
Occasional local travel for outreach or community support (valid driver’s license preferred).
Must be able to lift up to 10 pounds as needed.
