Job Overview
The Adult Partial Care (APC) program assists individuals who have severe and persistent mental illness with developing the skills and support necessary to live, learn and work with the least amount of intervention from the behavioral health system. The Adult Partial Care program offers comprehensive, individualized and quality behavioral health treatment within a group setting, designed to promote maximum independence in the least restrictive setting.

Adult Partial Care is a full-day service that provides individualized care for people with serious and persistent mental illness. Our goal is to treat individuals challenged by mental illness with compassion, concern and therapies that can help maximize their quality of life and empower them. Working together, our caring, experienced team of psychiatrists, nurses, therapists, case managers and technicians provide individual care for a wide range of disorders such as, schizophrenia, bi-polar, serious depression and co-existing addictions. Our Adult Partial Care program offers the best possible care in a supportive environment through therapeutic approaches and activities specially designed for individuals with mental illness. We'll work with you and your loved ones over the course of treatment to help you achieve your personal goals. Our board-certified psychiatrists will meet with you to talk about your psychiatric concerns and make recommendations for designing a treatment plan that meets your needs. We'll also provide case management services that can help you access important benefits to which you may be entitled. Free transportation and lunch are provided by the program.
Key Responsibilities:
The counselor plans and provides group services around consumers IRP treatment needs. Services include engagement strategies, activities to meet meaningful goals over time, Illness Management and Recovery Program, skill development, prevocational services, counseling, learning activities, social/leisure services; as well as offsite interventions. In addition to individual and group support services, counselors are responsible for maintaining authorization timeframes, intake and daily documentation, and service planning. Demonstrates competent written and verbal communication with referral sources and external customers. Attentive and responsive to customer needs in a helpful, supportive and timely manner. Works collaboratively with coworkers and agency staff to effectively communicate and problem-solve. Meets obligations of the program (contractual, licensing, Medicaid, etc.). Follows program policies and procedures and encourages the same in others. Educates self on issues impacting the program population. Operates conscientiously in accordance with program budget (i.e., use of client engaging funds, office supplies). Associate's degree and two years' experience in providing human services, or five years of human service experience, or possess a relevant professional credential, such as Certified Psychiatric Rehabilitation Practitioner, bachelor's degree in social work, certified rehabilitation counselor, certified alcohol and drug counselor, bachelor's degree in rehabilitation, or Bachelor of Science in nursing. Must have and maintain a valid driver's license with an acceptable driving record Must be able to exercise independent thinking and good judgment under all circumstances. Must be able to analyze work, set goals, develop plans and utilize time effectively and efficiently. Must have and maintain a valid driver's license, use of an insured vehicle and an acceptable driving record. Bachelor's degree in a human services field
Additional Information:
Starting Compensation: $18.27 per hour Sign on bonus: $750 Bilingual-Spanish differential: $1.50/hourly What we provide:
