The case manager assistant works under the direction of the RN case manager and the Social Worker to help facilitate the transition of patients to acute-care facilities. The position is responsible for supporting care coordination and may interact with insurance companies, home care agencies, skilled nursing, durable medical equipment, and transport agencies. Skills /Knowledge / Abilities
Computer skills: Excel, SAP, MS Office incl. Outlook, Excel, Word; AMION. Supervisory Responsibilities? No
Preferred Education/Experience
