Job Summary
Facilitates family reunification or alternate placement for at-risk youth and their families through crisis intervention, family counseling, stabilization, parenting skills, case management and providing community linkages and resources through a strength based approach utilizing evidence based practices.
Essential Duties/Responsibilities
Provides family therapy and case management services to shelter clients and their families with the goal of reunification. Works with Social Services, Probation, CBO’s, Schools, and other involved professionals in youth and family’s life. Develops treatment plan covering all life domains using TCP. Provides crisis intervention when needed. Provides on-call support when needed. Provides Aftercare services to youth and family. Maintains case history records, progress notes, and other documentation as requested. Works closely with Residential Counselors in supporting youth and family. Maintains clients file. Evaluates youth’s physical and psychological makeup to determine needs. Counsels youth and parents, guardians, foster parents or institution staff concerning adjustments to home situation, defining house rules and parent-child expectations, interactional behavioral modification or referrals as needed to meet family or individuals needs. Must be able to communicate effectively, get along with coworkers and management and deal effectively and professionally under pressure. Attends CFT's, FSP's (wrap team meetings) and weekly clinical reviews. Provides transportation for child to appointments when needed. Accompanies youth to Court when involved in Juvenile Justice system and prepares report for Probation Officer several days prior to Court date. Knowledge of or ability to learn Motivational Interviewing, Cognitive Behavior Therapy, Trauma-focused CBT and Transformational Care Planning, Trauma Informed Care. Documents ongoing client interactions on AWARDS and other databases as required. Participate in agency Program Quality Improvement activities and initiatives. Participate in agency Safety initiatives and proactively report safety issues to management immediately. Other duties as assigned.
Education And Experience
Bachelor’s Degree in counseling, social work, or related field; or four to ten years related experience and/or training; or equivalent combination of education and experience. Experience working with culturally diverse youth required. Must have possession of current driver’s license, access to a vehicle and personal automobile insurance. Current CPR and First Aid Certification required (or complete within 30 days of hire). Fingerprinting/background check as required by the Department of Justice and/or Title XXII regulations. TB test required. Personal or lived experience with housing instability, social services, systemic challenges, or the criminal/legal system is valued.
Required Skills/Abilities
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of clients or employees of an organization. Some positions require bilingual communication skills. Bilingual Spanish speaker preferred. Ability to work with basic mathematical concepts as required for reporting. To perform this job successfully, an individual should have knowledge of Internet software; Spreadsheet software and Word Processing software. Internet research skills are a plus. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Physical Demands
While performing the duties of this job, the employee is regularly required to sit; stand, use hands; reach with hands and arms and talk or hear. The employee is required to stand and walk, as well as walk up and down stairs. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close vision, color vision and ability to adjust focus.
Work Environment
Dynamic work environment that requires responding to various demands and supporting team in addressing needs of the client. Fast paced environment that will require frequent and often shifting of direction and focus.
Salary Range
$25.00 to $27.00 per hour
