Ensures patients have been cleared for specialty services office visits and serves as the point person for all hospitals, departments, and other health care facilities regarding provider orders and referrals. Provides support to clinical staff to facilitate the administrative components of clinical referrals for various services. Collects pertinent information from insurance carriers, financial counselors, and other ancillary staff to verify the patient’s financial obligations for services provided. Processes paperwork for new patients efficiently and in accordance with protocols. Verifies insurance coverage, obtains authorizations when needed, and resolves pre‑certification, registration, and case‑related concerns prior to a patient’s appointment. Tracks and supports patients when they obtain services outside the practice (e.g., emergency departments, hospitals and other health care facilities). Monitors and meets PCMH referral processes. Enters referrals and records communications, actions, and other data in an information system, ensuring that all referral requests are properly signed and filed. Coordinating care—assists with specialty appointments and obtains needed tests recommended by specialists. Communicates with other team members and referral teams constructively to reduce conflicts and enhance issue resolution. Acts as the system navigator and point of contact for patients and families, providing direct access for questions and concerns on orders and referrals. Collaborates with other team members to provide care management services for high‑risk patients. Obtains insurance referrals for patients, secures prior authorizations for diagnostic imaging, and sends referrals to specialist offices for cardiac and preventative screenings. Qualifications

Education

Associate’s degree or equivalent knowledge in an ambulatory environment. Experience

Five years of experience working with referral processes. Familiarity with EPC EHR system. We take great care of careers.
