"Navitas Healthcare, LLC" is seeking a dedicated
Licensed Master Social Worker (LMSW)
to provide professional social work services within an
Adult Comprehensive Psychiatric Emergency Program (CPEP) . This role involves performing patient assessments, crisis intervention, discharge planning, and care coordination in a dynamic behavioral health environment. The LMSW will work closely with a multidisciplinary team to deliver compassionate, patient-centered mental health services. Responsibilities Conduct psychosocial, crisis, and risk assessments for adult psychiatric patients. Develop and implement individualized care and discharge plans with the healthcare team. Deliver crisis intervention, counseling, and emotional support to patients and families. Collaborate with physicians, nurses, and community resources to ensure care continuity. Maintain accurate and compliant clinical documentation per hospital and state standards. Participate in interdisciplinary team meetings, case conferences, and treatment planning. Advocate for patient rights, safety, and access to community-based resources. Handle high-stress psychiatric situations with empathy, professionalism, and sound judgment. Uphold ethical practice standards, confidentiality, and patient-centered care. Stay informed on current best practices in social work and crisis intervention. Qualifications Education:
Master's Degree in Social Work (Required) Experience:
1+ year of hospital-based LMSW experience (Adult CPEP or psychiatric emergency preferred) Licensure:
Active NYSED LMSW License (Primary Source Verification Required)
