For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens, and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness.
Our program helps individuals and families move beyond emergencies to more stable and stronger home lives. COS encompasses the following three components.
Health and Wellness Services engages the community in interactive health and wellness workshops, support groups, events and resource fairs that cover daily needs including healthy living, COVID 19 and parenting.
Community Outreach Centers (formerly walk in center). Our Outreach Specialists assess the individuals needs and respond with information, resources, and limited financial assistance.
Food Resource Services operates a network of 20 food pantries (in partnership with parishes in the Diocese of Brooklyn). Our network helps distribute more than 3 million meals to 300,000 people. Meals include shelf stable items, dairy, meats and fresh produce. STATEMENT OF THE JOB: Under the direction of the Program Manager, the Case Manager I render assistance to WIPA clients through comprehensive case management services and workforce development counseling. The Case Manager I work in collaboration with the Program Manager and internal agency service providers.
Target, engage, and conduct outreach to students.
Provide case management services, assessment and intake of students to determine eligibility for services.
Carry a caseload and keep client data base up to date.
Maintain case records and program documentation.
Work with the client to assess strengths, needs and resources; develop an individualized service plan (ISP) and follow up.
Provide advocacy, referrals, and services for students benefit.
Develop and manage an Individualized Family Service Plan.
Assess the needs of the whole students/family including their basic needs.
Ensure that referrals are made to appropriate internal and external services.
Follow-up on referrals and respond to all referrals made to you.
Maintain linkages to ensure documentation and verification of participant outcomes.
Participate in professional/agency community activities and training opportunities as assigned.
Performs other related duties as requested or assigned.
QUALIFICATIONS:
Bachelor’s degree in social services or a related field preferred or Associates Degree in social services or a related field and Family Development Training credentials and an additional two years’ experience working with the City System such as Public Assistance, child welfare, education, and housing.
One-year experience working with the City System such as Public Assistance, child welfare, education, and housing and FDC Certification.
Bilingual English/Arabic, English/Cantonese/Mandarin, English/Spanish, and/or English/ Russian preferred.
Ability to work flexible hours (evenings and weekends) as assigned.
Must be able to Frequently lifts and/or moves up to 20 pounds.
Able to travel to multiple locations within the five boroughs as needed.
