You’ll provide therapeutic intervention and social work services to patients and their families to enhance comprehensive, integrated, and uninterrupted care in the hospital and to facilitate continuity of care in the community. You will conduct patient assessments, develop care plans, coordinate with providers, and serve as a patient advocate. The Social Worker collaborates with the psychiatrist and interdisciplinary team to develop and manage treatment plans, liaises with the hospital, community, and insurers to facilitate access to clinical treatment, and provides social services to both adult and child/adolescent patients. Services include, but are not limited to, bio-psychosocial assessments, discharge planning, individual and family education, and group therapy. Social workers provide individual, conjoint, group, and family psychotherapy; and crisis intervention as needed. Ensures the quality of care is safe, effective, patient-centered, timely, efficient, and equitable. May contribute as a field instructor for social work interns and participate on designated committees. Responsibilities

Master's Degree in Social Work from an accredited school. Clinical knowledge and analytical abilities to formulate effective social work care plans. Ability to perform a wide variety of professional social work services. Ability to effectively communicate with patients, families, and hospital personnel. Certifications/Licenses

Current Licensed Master Social Work (LMSW). Job Details
