Job Summary The Medical Social Worker provides brief therapy regarding improved management adjustment to illness and compliance with medical regime. They facilitate coping, assist with social and emotional illness, and strengthen the family support system to improve patient response to treatment. This role is a core member of the Hospice Interdisciplinary Team.

Essential Responsibilities

Makes initial assessment of terminal and multi‑problem cases and consults on cases with agency staff via case conferences or individually, including nursing, therapeutic, social services, homemaker‑home health aides. Also consults with other agencies by participating in interagency conferences and attending case conferences, including discharge planning.

Evaluates situation of the patient/family and develops social work plan of care. Provides ongoing social work services to assist patient/family with personal and environmental difficulties while referring them to appropriate community resources when needed.

Prepares clinical/progress notes and discharge summaries in accordance with licensure requirements.

Qualifications Education

Master’s degree in social work required.

Current license to perform as a medical social worker in the State of Connecticut required for Hospice program only.

Valid Connecticut Driver’s License and proof of current insurance coverage required.

Experience

Minimum of three years post‑graduate experience in social work with one year in a health‑care related setting preferred.
