Schedule & Location Shift: Monday–Friday, 9 AM–5 PM. Hours: 35 per week. Office: Manhattan (2 days a week) with a minimum of 3 days traveling to various locations in the Bronx and Manhattan. Additional travel may be required for emergencies.

Community Guardian Program Overview JASA assumes the legal role of community guardian for adults who have been judged legally incapacitated by the courts and require a legal guardian. Through assessment, comprehensive service planning, regular visits, ongoing support, and regular reports to the courts, JASA ensures that the needs of these individuals are met.

Essential Functions (Responsibilities)

Conduct initial assessments and monthly in‑home assessments of the social and emotional needs of the client and their family.

Screen the client for program eligibility, conduct triage for emergency interventions, ascertain short‑term needs, and provide appropriate information and referral services.

Develop and implement written service plans to provide a full range of social services.

Arrange for direct provision of services such as homemaker, home health aide, public assistance, Medicare, Medicaid, emergency cash relief, legal aid, protective services, vocational placement, medical and psychiatric examination and therapy, housing, etc.

Disburse monies as issued through JASA relief funds or client financial management services to secure needed goods and services.

Review the guardianship court order and ensure that all court‑ordered services are in place.

Advocate on behalf of the client.

Refer to and maintain a cooperative relationship with other community agencies to meet the needs of clients.

Seek appropriate guidance from the supervisor in situations requiring clarification and consultation, especially for cases that meet the high‑risk criteria.

Represent and interpret the agency at community conferences and meetings.

Participate in training and unit meetings.

Complete required case records, reports and statistics within mandated timeframes, utilizing computers and mobile devices as required.

Handle special assignments and duties as assigned.

Required Skills & Qualifications

Graduate of an accredited college or university with a Bachelor’s degree in Social Work, Sociology, Psychology, or Human Services, or equivalent.

Bilingual in Spanish and English.

1+ years of case management experience (strongly preferred).

Demonstrated organizational and time‑management skills.

Ability to travel to clients’ homes using mass transit and to navigate any barriers in clients’ residences.

Computer literate and able to learn and operate software.

Understanding and appreciation of the roles of race, religion, ethnicity, sexual preference, and individual values as they relate to serving clients and families.
