Social Worker – St. Croix Hospice St. Croix Hospice provides compassionate end‑of‑life care to patients and families in the Michigan region.

The Social Worker will offer emotional and practical support, coordinate with the interdisciplinary team, and connect patients and families to community resources.

Position Overview The Social Worker is an experienced member of the patient care team who delivers psychosocial support based on the patient’s individual Plan of Care.

Working under the supervision of a Master’s prepared Social Worker, the role includes assessment, counseling, bereavement care, and collaboration with the clinical team to ensure comprehensive patient and family support.

Essential Functions & Skills

Assess psychosocial status of patients and families, communicate findings to the RN and interdisciplinary team, and provide evaluation in patients’ residences.

Perform social evaluations covering family dynamics, caregiver abilities, communication patterns, suicide risk, neglect, and abuse; plan interventions based on findings.

Counsel patients and families as needed for stress and other identified coping difficulties; provide crisis intervention when necessary.

Offer ongoing bereavement support to individuals identified as moderate or high‑risk.

Assess cultural needs of patients and families, educate the interdisciplinary team, and address communication and role of family issues.

Work under the supervision of a Master’s prepared Social Worker (MSW).

Maintain clinical records for all patients referred to social work.

Educate patients and families, assist in preparing advanced directives, and provide information/referral services regarding practical and environmental needs.

Serve as liaison to community agencies and develop contracts with public and private agencies as resources for patient and personnel.

Assist physicians and team members in understanding significant social and emotional factors related to health problems and death/dying issues.

Assist bereavement leadership in maintaining a program that meets the needs of families/caregivers for 13 months following the death of a patient, ensuring compliance with all state, federal, and CHAP regulatory requirements.

Ensure the initial bereavement assessment is completed or assigned to the appropriate team member in a timely manner.

Assist the Chaplain in making condolence calls to bereaved as needed.

Assist with maintaining, organizing, and updating bereavement support group schedules.

Assist family and patient in planning funeral arrangements, financial, legal, and health care decision responsibilities.

Maintain compliance with all HIPAA regulations and requirements; treat all patient information as confidential and utilize protected health information on a minimum necessary basis.

Qualifications

Must have a bachelor’s degree in social work from an accredited institution.

Current licensed in the state(s) of practice.

Licensed Bachelor Social Work (LBSW), Licensed Master Social Worker (LMSW), Limited Licensed Master Social Worker (Limited LMSW), Licensed Clinical Social Worker (LCSW), Social Service Technician (SST), or Limited Social Service Technician (SST).

Social Workers may have compact licenses.

Ability to pass background, consumer report, and drug test.

Valid State Driver’s License, access to a vehicle in good working order, and an acceptable driving record.

Preferred

Master’s degree in social work, preferred.

1+ years of social work experience in health‑care settings.

Physical Requirements

Push, pull, and lift/carry up to 12 pounds; occasionally up to 20 pounds.

Ability to adapt to a variety of work conditions to ensure the highest quality of patient care.
